SilverStay helps hospitals improve patient discharge coordination. Technology Rivers developed AI-powered hospital discharge planning software that automates patient data collection, identifies potential discharge barriers, and suggests next steps for care teams.
The Discharge Navigation Platform (DNP) combines patient information, AI insights, and daily tasks in one secure web application. It helps discharge planners, care navigators, and social workers spend less time searching records and more time addressing patient needs.
Built with HIPAA requirements in mind, this work continues a long-term healthcare software partnership.

The Challenge
Hospital discharge planning requires reviewing patient records, identifying obstacles to discharge, and coordinating next steps across physicians, social workers, case managers, therapists, and other care professionals.
Much of this work involves manually searching through clinical documentation, often called “chart digging.” According to SilverStay’s product manager, hospital staff may spend 30 minutes to more than two hours a day reviewing records and gathering information.
SilverStay had tested its approach with partner hospitals, but early workflows still relied heavily on Google Sheets. Staff imported data, moved it between files, and manually prepared summaries.
Electronic health record (EHR) systems made patient information available, but SilverStay needed hospital discharge management software that could transform complex information into useful next steps.
The goal: Reduce manual work, clarify discharge barriers, and help teams focus on patients requiring attention.

Automated Patient Data Intake
The platform receives hospital patient files three times a day through secure file transfer. Incoming information is processed and organized automatically, reducing copying and movement between spreadsheets.
This supports healthcare workflow automation by reducing repetitive manual data handling.
AI-Powered Discharge Barriers and Next Steps
AI reviews clinical documentation from physicians, social workers, case managers, therapists, and other healthcare professionals to identify potential medical and non-medical barriers to discharge.
This distinction helps care navigators identify issues they can address directly and concerns that should be discussed with clinical staff. The system also suggests possible next steps.
AI-Generated Meeting Summaries and Follow-Up Tasks
Care teams can upload transcripts from discussions of complex patient cases. AI summarizes conversations, identifies follow-up actions, and adds tasks to daily patient lists.
Length of Stay Monitoring
The platform tracks hospital length of stay (LOS), helping teams recognize patients who may need additional attention.
Patient Readmission History
When patients return, information from previous stays can be linked to their new cases, providing historical context without starting from scratch.
Role-Based Access
Supervisors and care navigators receive access appropriate to their roles. Amazon Cognito supports authentication.

How We Built It
Technology Rivers developed the AI-powered hospital discharge planning software in two stages: automating patient data intake and introducing AI capabilities to analyze clinical information.
Stage 1: Replacing Manual Spreadsheet Workflows
Technology Rivers implemented automated SFTP-based intake so partner hospitals could send patient files securely. The system processes incoming data and creates a centralized foundation for the application.
Stage 2: Adding AI to Discharge Planning
We integrated Claude through Amazon Bedrock to analyze clinical notes, identify potential barriers, summarize relevant information, and suggest next steps. Insights appear in a responsive interface designed for care teams.
This work brought together AI & Machine Learning Development and Web Application Development to support SilverStay’s hospital discharge workflows.
Solving Complex Healthcare Data Challenges
Discharge readiness cannot always be inferred from one field or note. Different sources must be considered together, and some information applies only under certain conditions.
Technology Rivers worked with SilverStay’s product manager to implement the data mapping rules and improve the way clinical information is processed and presented.
Iterative Development and Quality Assurance
The teams collaborated through short development cycles and regular meetings. Requirements came from Slack, Google Docs, spreadsheets, and team discussions, and are now managed in Planwright. Developers clarified details, translated requirements into tasks, and tested changes before release.
After the version 1.0 rollout, frontline user feedback helped guide improvements to the dashboard and daily workflows.

Healthcare Security and Compliance
The platform processes protected health information (PHI). Security and privacy considerations informed AWS architecture, secure data intake, authentication, and role-based access.

Results & Impact
